Flu-like symptoms is a general term used to describe someone who is calling with just that, flu-like symptoms. So when the person has multiple complaints such as nausea, vomiting, diarrhea, dehydration, not being able to keep things down, weakness, malaise, vertigo, sore throat, fever, etc. that's the "flu-like symptoms" type of thing. It's sort of a catch all. And we get called for that more than you know... lol. But seriously, in the elderly it can certainly be dangerous.

As for the private ambulance thing, it just means a private ambulance company. For example, AMR is a private commercial ambulance service, but LAFD is not a private company, they are city.

As for transporting with or without l&s, it's entirely up to the medic or tech in the back whether they want to transport with or without. I could have two different patients with "flu-like symptoms" and transport one with and one without. It just depends. I look at whether or not this person needs a hospital immediately, and do I want to risk running lights and sirens. I mean, if I have a person who is having a STEMI, obviously I'm not going to waste any time and I want them to the hospital yesterday, so I'll definitely go with lights and sirens. But if someone has been sick for a few days and nothing has really changed within those few days, I'm not going with lights and sirens.
So nothing specifically dictates when you have to or don't have to transport with lights and sirens, but there are instances where it's common sense, such as a STEMI or a stroke patient with an onset of less than (usually) 3 hours.
